If I choose to live here, what will my home be like?

Home-Like Environment: We take pride in the fact that residences within the Lori Knapp Companies system are "homes". Although we're a business, we work hard to keep a home-like environment. We strive to have the home be the least institutional as possible. Furnishings are provided, but you are encouraged to bring your own personal items & belongings. That way you can personalize your living space and can encourage remembrances of times past. When you walk into our front door you feel at home. Gracious Way has beautifully decorated common areas & landscaped outdoor areas. You can smell home baked cooking throughout the day.

Accommodation Options:

• Private & couple (double occupancy) accommodations are available. Additionally unique apartment-type accommodations are available if you require minimal assistance.
• Private bathrooms
• Walk-in Jacuzzi-type tub
• Plenty of room to walk around and exercise

Safety/Security: We at Gracious Way consider your safety to be very important. Although we are not a locked facility, an alarm will activate if someone chooses to wander. Additionally, we feel that we maintain safety by making certain of adequate staffing ratios and by having weekly quality reviews to assure that we are complying with established guidelines.

Apartments: We provide a unique opportunity for couples and family members who wish to be close to their loved-one with Alzheimer's or dementia. These apartments are spacious and gracious. Our apartments provide the opportunity for a couple to move in and receive care while still remaining together. Levels of care and help needed are determined on an individual basis.

Getting To Know You: To best serve you, we wish to know you first. Through simple discussion, we can begin the process of learning about you and your needs. We work with you, your family, and other experts to meet your individual needs. All aspects of your care are meticulously assessed and a plan is developed specifically for you.

How much will it cost me to live here?

Our Rates: Whether you are privately paying or accessing services through a managed care organization, insurance, or other funding, the Lori Knapp Companies will work with you to help you receive the best care you deserve at costs you can afford. Charges are structured on a per day, per care level formula and our rates are comparable to nursing homes and other related facilities. If you are eligible for government funding, we will refer you to the right agencies for help. These include the Aging & Disability Resource Centers (ADRC) or county human service agencies.

Who will be the folks that will serve me and my needs?

Your Team Of Caregivers: Your Gracious Way team will provide consistent, quality cares that encourage dignity and safety. They are members of the Alzheimer's Association and are intensely trained in Alzheimer's and dementia care. The core team includes a staff of certified personal care workers, nursing assistants, and an activities director. Directing the team is a RN supervisor, and a full-time residential coordinator who oversee all activities at Gracious Way. To promote consistency and quality at Gracious Way, staff trainings address: safety & well being, privacy & confidentiality, client rights, prevention of abuse and neglect, medication oversight, diabetic care, specific dietary needs, non-violent crisis intervention for persons with challenging behaviors, seizures, promoting health and wellness, needs and assessment, supervised on the job training, and additional state-mandated trainings. If you have special care needs, your staff will be trained to meet those needs.

Team Schedules & Ratios: Having adequate team members on-site to serve you is the key to success. This includes a caregiver being awake during the night to address any concerns that arise. Staffing patterns are intended to be flexible to meet your changing needs.

RN Involvement: Because we know that your medical needs are important, a RN visits Gracious Way weekly. The RN will get to know you and ensure that your staff are competently trained to meet your ongoing needs.

If I choose to live at Gracious Way, what will a typical day be like for me?

Activities: Activities are an important part of anyone's daily lifestyle. Knowing that choice is important, we will encourage your involvement in meaningful social, cultural, and recreational activities. These can include community outings, walks, shopping, puzzles, magazines, crafts, daily current events, fitness/exercising, cooking activities, and other activities that interest you. You will choose the activities of your liking.

Choices: Having the ability to make choices throughout the day is important and necessary. Your independence is encouraged in your choice of daily routine and activities, as appropriate. Your caregivers at Gracious Way will work with you to encourage a safe and meaningful routine.

Visitors: Keeping in touch with your family and friends can be a vital element. We have a family room available for private time or a small party. Your caregivers will help coordinate your visitation time.

Meals & Diet: While Gracious Way has established meal times, when possible you may choose to have your meals at different times. Your dietary needs are taken in to consideration and we will provide special meals such as vegetarian, low salt, diabetic and other diets.

Medication Oversight: Any medications and therapies prescribed to you will be managed and administered by trained Gracious Way staff. Our medication practices follow the state of Wisconsin health guidelines.

Housekeeping: You are encouraged, but not required, to take part in housekeeping activities.

My future is uncertain. Can I "age in place" at Gracious Way?

Supporting You Through The Aging Process, Including End Of Life Care: A founding premise of the Gracious Way Assisted Living Facility is what's known as "aging in place". To us this means that we will care for you until end of life, unless your medical providers deem otherwise. We welcome hospice into Gracious Way and are willing to go the extra mile to make you and your family comfortable.
